How do I gain access to the patient portal?
You will need to create an account and receive a pin for EACH child to gain access.  You can receive a pin either by email or by a paper print out in the office. Check out the "Quick Setup Guide
" to assist you depending how you received the pin.

What are the various options under the navigation panel and quick access buttons?

Do you have an instructions around how to navigate and/or utilize the patient portal?

How do i send a message or attach a document for my child and/or provider?

*Please only send PDF forms. We cannot complete forms sent in another format. Read this quick instruction guide to find out how to upload forms to be completed for your child, and how to upload a copy of your new insurance card. Use this method to send us a Camp form, Sports form, Daycare form, PT/OT/Speech therapy forms, Medication forms for school, 504 or IEP forms, Lead screening forms (please fill out answers before sending to us), etc.*

How do i find a requested document, completed form or immunization record that was sent back to me?

What if i am not able to access my portal account or concerned it has been compromised?
This Patient Portal is provided by PHA. for the exclusive use of its patients and authorized parents, legal guardians, and other caregivers. If you believe that the security of your account has been compromised, please notify us immediately so we can reset your credentials.

Is it safe to access my child's records online?
All access to Electronic Medical Records is provided on secure pages that are controlled through passwords known only to the parent and appropriate staff. To protect the privacy of your records, from this point forward, all data communication between your computer and our server is encrypted. You can verify the encryption status by looking for a lock icon in the status area of your web browser.
