A claim will be submitted to your insurance at the time of service. We do collect ALL patient responsibility whether it is copay, deductible or co-insurance on the date service is provided.
If we are not a contracted provider for your insurance, or if your plan does not have out of network benefits, we will file a claim with your insurance company, however you will be responsible for the entire charge at the time of service.
