1.) 1st Newborn Appointment please bring the discharge documents from the delivering hospital with you to your initial visit.
2.) New Patients must provide Immunization records at the initial visit or they can be sent to [email protected] or faxed 972-562-0781. Immunization records are required for initial visit.
