2024-2025 School Sports Physicals

Attn Parents!!

Now is the time to schedule your child that will be participating in school sports, band, cheer or dance for the upcoming school year 2024-2025.  UIL requires all students in grades 7th-12th to have a sports physical by a physician in order to participate by the start of the school year in August.   The appointment availability is limited and we encourage you to call and schedule now for over the summer appointments.    

Please bring the sports physical form that is issued by your child's school district, with the parent portion already completed and the physician with complete and sign the form during the appointment.
