Meet-N-Greet
Welcome to our "Meet-N-Greet" registration form. This meet-n-greet option has weekly open time slots to anyone seeking a pediatrician in the surrounding Collin, Denton & Grayson areas.
If you are a newly expectant mother, looking for a new pediatrician, or just recently moved to the area, you can chat one-on-one with one of our 4 doctors that are currently taking on new patients. They are ready to answer any of your questions, polices, or concerns you may have as well as share a bit of their extensive knowledge with you!
The meet-n-greet is preferably scheduled at 8:00am or 1:00pm. This gives us time to tour the office and stop by each provider for a quick introduction and any questions you may have.

Pre-Natal/New Baby Visits

At your initial visit, please remember to bring ALL of your discharge records or medical records with you to the visit. If you have any questions, please feel free to contact us and we will be happy to go over it with you.
!!Remember to add your newborn to your insurance within 30 days!!
Please contact our office to schedule an appointment to visit with one or all of our physicians. This appointment provides a time to meet the physicians one on one to discuss your concerns or questions as a new parent and the expectations for both your family and our providers. We also offer a tour of our office and introductions to our staff, just to get a look at what to expect.

What is Required?
Please complete the online patient registration form prior to your appointment. Completed forms are electronically sent to the front desk staff.
1.) 1st Newborn Appointment please bring the discharge documents from the delivering hospital with you to your initial visit.
2.) New Patients must provide Immunization records at the initial visit or they can be sent to [email protected] or faxed 972-562-1170. Immunization records are required for initial visit.
